Purple Card:

All prospective athletes need to have purple cards stamped by the nurse and turned in to their respective coaches or the athletic director, Mr. Cerone. Purple cards can't be stamped without an updated physical. A new purple card is needed for each season.

Concussion Forms:

Concussion forms are necessary for all student athletes. This warns parents and students of the possible injuries a student may face playing sports. These forms must be signed before a student can participate. Forms are available during the registration process.
